Невиношування вагітності в Україні як проблемна складова демографічної ситуації

Анотація
Невиношування вагітності (НВ) є суттєвим чинником репродуктивних втрат і важливою складовою демографічної кризи в Україні. За оцінками, 15–20% клінічно підтверджених вагітностей завершуються самовільним перериванням. В умовах зниження народжуваності навіть незначне зростання частоти втрат вагітності чинить критичний вплив на демографічні показники. У статті проаналізовано статистичні дані, етіологічні чинники й визначено основні напрями профілактики НВ.
Мета дослідження: аналіз показників НВ в Україні та визначення їхнього впливу на кризову демографічну ситуацію.
Матеріали та методи. Проведено двоетапне ретроспективне епідеміологічне дослідження. На І етапі аналізували частоту передчасних пологів у 2025 р. та фактори ризику, що найбільше асоціюються з передчасним народженням; на ІІ етапі – кількість передчасно народжених дітей та їхню смертність в акушерських стаціонарах або закладах охорони здоров’я педіатричного профілю (після переведення немовлят із зазначених закладів) у 2025 р. Застосовано бібліосемантичний, аналітичний, епідеміологічний, статистичний і концептуальний методи дослідження.
Результати. За результатами дослідження продемонстровано рівень впливу складових проблеми НВ на демографічну ситуацію в Україні. Основними причинами НВ визначено недотримання унормованих принципів регіоналізації перинатальної допомоги й організації неонатальної допомоги, недостатнє застосування профілактичних технологій та недосконалість маршрутизації пацієнток із загрозою НВ.
Висновки. НВ є важливою складовою демографічної кризи в Україні. Воно спричиняє значні демографічні втрати (до 40 тис. випадків втрати вагітності щорічно за віддаленими результатами), а в умовах критично низької народжуваності його вплив посилюється. Для України характерні додаткові фактори ризику, зокрема вплив війни, стресу та обмежений доступ до медичних послуг. Зниження частоти НВ можливе за умови комплексного підходу, що включає медичні, соціальні й державні заходи. Підвищення уваги до цієї проблеми є необхідним для збереження репродуктивного потенціалу нації. Зниження частоти НВ є реальним резервом підвищення народжуваності.
