Проблеми клінічних досліджень в Україні, пов’язані з внутрішньо переміщеними особами

Анотація
Питання якості клінічних досліджень (КД), за які в Україні відповідають переважно етичні комітети і державні органи влади, розглядаються з метою підвищення доступності та якості досліджень за участю внутрішньо переміщених осіб (ВПО).
Мета дослідження: систематичний огляд основних проблем, які виникають при проведенні КД в Україні у зв’язку з внутрішнім переміщенням населення після 2014 р. і, особливо, після повномасштабного вторгнення росії у 2022 р.
Матеріали та методи. Проведено аналіз літератури, включно з національними документами та міжнародними публікаціями (Всесвітня організація охорони здоров’я (ВООЗ), Управління Верховного комісара ООН у справах біженців (UNHCR), статті в рецензованих журналах, огляди й практичні настанови), матеріали українських регуляторних органів і профільних аналітичних звітів. Для вибору джерел використано бази даних PubMed, PMC, офіційні сайти ВООЗ, Міністерства охорони здоров’я (МОЗ) України, Державного експертного центра МОЗ України та аналітичні публікації. Ключові питання аналізу: які бар’єри для участі ВПО у КД; як переміщення впливає на якість даних; які етичні/регуляторні рішення використовуються; які практичні рішення рекомендовані. Частина висновків підкріплена актуальними звітами про стан КД в Україні станом на 2023–2024 рр.
Результати. На основі аналізу літератури й публічних документів розглянуто логістичні та інфраструктурні бар’єри КД в Україні, методологічні проблеми й валідність даних КД, етичні та правові виклики, які виникли під час війни, регуляторні та адміністративні обмеження, а також вплив соціальних і психологічних факторів на ефективність КД. Запропоновано рекомендації для дослідників, спонсорів, етичних комітетів та державних органів із метою підвищення доступності і якості досліджень за участю ВПО в Україні.
Висновки. Включення ВПО у КД вимагає балансування потенційної користі й ризиків: важливо забезпечити доступ до лікування та одночасно зберегти етичні стандарти і якість наукових даних. Розробка гнучких протоколів, зміцнення регуляторного поля, етичні гарантії, міжсайтове співробітництво та інтеграція психосоціальної підтримки – ключові кроки для забезпечення безпечного й етичного проведення досліджень за участю ВПО. Скоординовані дії спонсорів, регуляторів, етичних комітетів і гуманітарних структур можуть зменшити негативний вплив внутрішнього переміщення на КД та захистити права й здоров’я вразливих пацієнтів.
