Сучасний стан проблеми патогенезу і фармакотерапії варикозної хвороби в акушерстві

Анотація
У статті представлений огляд сучасних вітчизняних та зарубіжних видань з питань етіопатогенезу та фармакотерапії варикозної хвороби (ВХ) під час вагітності. Розглянуто питання щодо поширення ВХ у вагітних, особливості венозної системи за фізіологічного та патологічного перебігу гестації, сучасні погляди на механізми порушення венозної гемодинаміки та її впливу на стан фетоплацентарного комплексу. Привернуто увагу до локальної активації системи гемостазу при пошкодженні ендотелію за наявності венозної гіпертензії під впливом активованих нейтрофільних гранулоцитів і моноцитів.
Проведений аналіз екстрагенітальної патології у вагітних з ВХ. Визначено роль дисплазії сполучної тканини у генезі ВХ та необхідності своєчасної діагностики і профілактики ускладнень перебігу вагітності та розвитку плацентарних порушень у жінок з ВХ, у визначенні маркерів дисфункції ендотелію. Зроблено акцент на ролі стану репродуктивної системи та хронічних вогнищ піхвової і цервікальної інфекції у прогресуванні ВХ та акушерських ускладнень, що пояснюється розвитком оксидантного стресу і тканинною гіпоксією та погіршенням передімплантаційних умов й перспективного сценарію виношування вагітності.
Представлені сучасна класифікація ВХ за характером морфологічних змін венозного русла й наслідків флебогемодинамічних порушень та клінічний клас захворювання вен нижніх кінцівок, що використовується у клінічній акушерській практиці. Наведені основні напрямки діагностики та консервативного лікування вагітних із ВХ в Україні (компресійна терапія, системна фармакотерапія та місцева топічна терапія), що відповідають світовим стандартам.
Висвітлено особливий вплив діосміну на редукцію симптомів ВХ та ендотеліальної дисфункції за наявності імунозапального пошкодження ендотелію та стінок вен на тлі оксидантного стресу. Цей вплив підтверджується поліпшенням стану внутрішньоутробного плода та перинатальних наслідків у вагітних з плацентарними порушеннями.
